SUBSTANCE USE SUPPORT
Regardless of the type of substance use, our experienced team of companions and coaches will coordinate and provide the right conditions for any specific needs. We understand the challenges of overcoming a drug or alcohol addiction and how to best manage the road to sobriety.
Mental health support

CAre MANAGEMENT
Ensuring care is organized and managed appropriately is crucial for recovery. Our experienced case managers excel at overseeing our clients’ safe keeping in an effective, communicative, and collaborative manner. Our case management team prioritizes the following:
Excellent communication with all caregivers
Coordination with medical doctors and therapists in an efficient manner
Working directly to create both short- and long-term goals for the required care
Effectively managing medication regimens
Supportive Aides
We offer 24/7 companions who assist the transition from treatment back to the home environment. They work with our clients to ensure they have the resources needed to be successful. They also train on how to manage triggers and find fulfillment in life.
Mental Health & Addiction coaching
Our mental health and addiction coaches learn about each specific case so they can customize a coaching plan that works specifically for the client. Upon deducing specified needs, they coach through structuring daily tasks and living more meaningful lives through service work, social programs, and more. By meeting individually two to five times per week, they provide specific tactics that help the individual find purpose in the day-to-day and advance larger life goals.
CARE TRANSPORTATION
We offer full transportation for patients needing a safe and secure way to travel to and/or from treatment and life events. We make sure they are safe, comfortable, and in expert hands throughout the process.
IN-PATIENT CARE SUPPORT
We collaborate with treatment centers and hospitals to offer a wide range of supportive services for in-patient care. If someone is receiving treatment at a facility or hospital, we work with them to understand their specific condition so we can provide any supporting care needed. We also offer one-on-one support for higher acuity in-patient clients.
Conservatorship Services
We provide services for helping families navigate the process of conservatorships. Through our years of experience, we have cultivated an extensive network of attorneys, psychiatrists, medical providers, and other professionals to help at every step of the way from start to finish. We understand it is a complicated and emotionally charged process. The needs and circumstances for each conservatorship is different in severity, approach, and individual path.
